The present study was undertaken firstly, to know the spectrum of lesions of the endometrium with respect to perimenopausal bleeding and clinical features and secondly, to determine the pick-up rate of endometrial biopsy for various pre-malignant and malignant lesions of endometrium.</p><p><strong>Material & Methods: </strong>This is a observational study with convenient sampling of 520 women of perimenopausal age group with AUB, presenting to gynecological OPD and wards of Department of Obstetrics and Gynaecology, R. D. Gardi Medical College and associated C .R. Gardi Hospital, Ujjain. The study was conducted from May 2012 to June 2014. All those subjects who fulfilled the eligibility criteria were subjected to clinical examination, ultrasonography, and histopathological examination. </p><p><strong>Results: </strong>Overall Sensitivity of clinical examination with respect to ultrasonography is 26.2%, 28.6% with respect to histopathology. Overall Specificity of clinical examination with respect to ultrasonography is 84.5%, 84.7% with respect to histopathology. Overall PPV of clinical examination with respect to ultrasonography is 68.0%, 62.5% with respect to histopathology. Overall NPV of clinical examination with respect to ultrasonography is 46.8%, 57.1% with respect to histopathology. <strong></strong></p><p><strong>Conclusion: </strong>The prime concern of this present study while evaluating abnormal uterine bleeding in women of perimenopausal age group is not to miss a significant cancerous lesion. The chances that such a lesion would be missed are rare, if we stick to the criteria for thorough clinical examination followed by ultrasonography and histopathology based on endometrial biopsy and usually no further expensive, cumbersome investigations may be necessary like saline infusion sonohysterography, hysteroscopy, enzyme histochemistry, hormone receptors. The complications that occur in the post operative period determines the outcome.</p><p><strong> </strong></p><p><strong>Materials and Methods:</strong> A comparitive study on Post operative complications in elective versus emergency laparotomies done in our Instituition (Thanjavur Medical college Hospital). The study period was of 2 years (August 2014 to July 2016). The two groups(elective and emergency) were followed up and studied for a period of two months and the post operative complications were recorded. The nature of study was quantitative. Participants were selected between the ages of 13-70 to keep the homogeneity of the group. The study comprised all patients who have undergone laparotomy in our institution during the study period. 79 people were undertaken for elective laparotomy group and 100 to the emergency laparotomy and the total respondents were 179.</p><p><strong> </strong></p><p><strong>Results and Conclusion</strong>: Among 179 respondents most of the respondents had Acute abdomen/peritonitis as an indication for laparotomy. 25.1% were in the age group of 46-55 years, 64.2% males and 35.8 % females. The surgery was performed for 1 to 2 hours in 64.2% of the respondents. Among 34.1% of them duration was less than 1 hour and more than 2 hours of duration 1.7% of the respondents. Post operative complications were observed in 53.2% of the respondents,70% of them were those who underwent emergency laparotomies Post operative vomiting and nausea was observed in 12.3% of the respondents. Commonly seen in those undergoing emergency laparotomy. Post operative fever was commonly observed in respondents who underwent emergency laparotomy. Post operative pulmonary complications were observed in 16.8% and also commonly with emergency laparotomy group. Postoperative wound infection rate was more in emergency laparotomy group(28 %). Enterocutaneous fistula was seen in 1.7% of the respondents and was only in those undergoing emergency laparotomy. Paralytic ileus was seen in 19% of the respondents with emergency laparotomies. 9% ,11%,3% of the respondents had anastamotic dehiscence, septicaemia and adhesive intestinal obstruction respectively in emergency laparatomies. 39.2% of the respondents who had right subcoastal incision on elective laparotomy had incisional hernia and none had in emergency laparotomy group. Although rare than its pulmonary counterpart, it in most cases is the initial diagnostic marker of this systemic infection. HIV infection predisposes a person to tuberculosis infection and reactivation of latent tuberculosis.<br />Aims: The study is aimed to find out the incidence, histo-pathological features and clinical spectrum of cutaneous tuberculosis in HIV positive patients.<br />Materials and Methods: A total of 15 cases of newly diagnosed patients of cutaneous tuberculosis attending dermatology out patient department of a tertiary care hospital in over a period of 1 year were included in the study. A through clinical examination and investigations including histopathological examination was carried out. Investigations were targeted towards finding a pulmonary or primary source of tuberculoisis.<br />Results: Scrofuloderma was the most common form of cutaneous tuberculosis seen in HIV positive patients, reported in 33% cases followed by Tuberculosis verrucosa cutis in 26%, papulonecrotic tuberculids in 13%, lupus vulgaris, military tuberculosis and lichen scrofulosorum, in 7% cases. The Mantoux test was positive in 83.33% cases. Characteristic tuberculoid granulomas were seen in 72.22% cases of lupus vulgaris, 42.86% cases of scrofuloderma and all cases of tuberculosis verrucosa cutis and lichen scrofulosorum.<br />Conclusion: Cutaneous tuberculosis in its various forms is still prevalent and awareness and a high level of clinical suspicion and histopathological confirmation is the key to early diagnosis and successful management of these cases.</p>2016-12-15T00:00:00+0530Copyright (c) 2024 International Journal of Medical and Pharma Research (IJMPR)https://journals.foundationspeak.com/index.php/ijmpr/article/view/860AN UNUSUAL CASE OF LEFT INGUINAL HERNIA STONE: AN INTERESTING AND INCIDENTAL FINDING2019-05-16T21:58:08+0530Jatin K. Bayeseditor@ijsss.comAbhishek Vermaeditor@ijsss.com<p>A 56 year old patient who present as a simple right inguinal hernia for last 18 years was admitted and planed for right hemioplasty. All routine investigations were within normal limit and patient was posted for surgery on next day. No ultra sonography or x-ray of abdomen wa~ done as diagnosis in case of hernia is made by clinical examination. During surgery, as it was a direct hernia so simple reducing the sac and placing a mesh over it was decided. But as the sac was freed from all the adhesion a solid and hard structure is felt against the thumb and right index finger. The sac is thus open to rule out any other pathology and silver off white stone was found just below the sac. The stone was removed; closure of sac was done followed by placing a mesh over it. The stone was send for biochemical analysis, which was later diagnosed as calcium stone. Patient was on regular follow up and no evidence of recurrence or infection was seen.<br />The main motive to present this case was to put stress that in a long standing case of inguinal hernia chronic, inflammation and i"itation may cause calcification and fibrosis of surrounding tissue which lead to formation of herniolithiasis (stone in hernia sac).</p>2016-12-15T00:00:00+0530Copyright (c) 2024 International Journal of Medical and Pharma Research (IJMPR)
