CLINICOPATHOLOGICAL STUDY AND SURGICAL AUDIT OF THE CASES OF APPENDICECTOMY

Abstract


Introduction : Acute Appendicitis is a most commonly diagnosed acute surgical condition of the abdomen in young males and females. Approximately 8% of population with male : female ratio of 1.4:1 (8.6% in males and 6.7% in females) will have appendicitis in their life time, with peak incidence as much as 70% of patients occurring between 10 and 30 yrs.(1)

This study is done to correlate Clinical Diagnosis with Post Appendicectomy pathological findings and to carry out Surgical Audit of cases of Appendicectomy performed.

Materials and Methods : A total of 100 cases, in the duration from October 2012 to April 2014 were studied. In detail complete history, clinical examination and few important investigation to confirm clinical diagnosis was carried out, appendicectomy performed and gross features of appendix followed by histopathological study were recorded to confirm and to type appendicitis.

Results : The Sensitivity, Specificity, for ultrasonography is 70.7%, 72.72% respectively, Positive Predictive Value is 95.3% and Negative Predictive Value is 23.5%, with a diagnostic accuracy of 71%, concluding that histopathological examination is a better and more valuable tool for diagnosing appendicitis.

Conclusion : Early diagnosis of appendicitis by a good clinical history and examination and its treatment by timely appendicectomy is must to reduce morbidity inspite of the normal investigations.
