PERFORATION IN ACUTE APPENDICITIS WITH LOCALIZED PERITONITIS VERSUS GENERALIZED PERITONITIS : A COMPARATIVE STUDY

Abstract


Introduction: Perforated Appendicitis (PA) is a common surgical emergency and many a time presents atypically with diagnostic dilemma. This is common in the extremes of ages. Correct decision making for the diagnosis is vital and if not treated in time, it has devastating effects on the patient with increased morbidity and mortality. 

 

Methods: 915 cases of Acute Appendicitis (AA) were treated in which 808 patients underwent emergency appendicectomy. PA with localized peritonitis(LP) was present in 71 patients; 54 patients underwent  open procedure  and  21 had  laparoscopic procedures. 30 and 4 patients had generalized peritonitis (GP) in open and laparoscopic procedures respectively.

 

Results: Generalized Peritonitis (GP) was significant in young and old age, low literacy and delayed diagnosis/ treatment. GP is defined if two or more abdominal quadrants were involved.  Localized peritonitis was commoner in males, inadequate delayed treatment, patient delay and patients from the hills. The morbidity and mortality were higher in patients having GP.
